CEU Forms Available
Paper copies of the Summer Institute CEU Verification Form are available at the check-in table. This form is for your own records and does not need to be turned in.
Instructions are included at the top of the document. For each keynote and breakout session you attend, circle the “1” in the CEU column. Each circled session equals 1 CEU. Please note that breakout sessions run at the same time, so you may only circle one breakout per time slot. At the end, add your totals for Day 2 and Day 3.
